You will need the following paperwork to be registered with the club and league:

For Tryouts:
*  Current USA hockey registration 9/1 thru 8/31(web site is located under the registration tab on the drop down menu)
*  A signed FZW Hockey Club camp and tryout permission form.
*  A player / parent information document completed.
*  Tryout fee. (check made payable to FZWHC)
*  A copy of the previous semesters report card to prove eligibility. 



For Registration after tryouts:
*  A copy of player's birth certificate.  Eligibility is partially determined by date of birth so we need a copy of this document.
*  A copy of the most recent report card showing the "semester" grades.  This will be the last report card of the previous school year.  We also collect the report card after first semester of the school year.  Eligibility is partially determined by credits earned each semester so this is a required piece of paperwork.  A player can become ineligible after first semester if proper credits have not been earned / passed. 
*  Mid States registration.  This will be done on the web sites provided under the Registration tab on the the drop down menu)
If you are listed as a Varsity or JV player you will register under one web site, if you are listed as a JVD or C team player there is a different web site.  Some players will register for both - see instructions under that tab. 
*  Signed Mid States emergency medical care and Consent to treat form - two copies (this is on our web site under Registration / forms)
