For the Mid States hockey league (Varsity and JV):

This is where you will register for the Varsity and/or the JV team. If you played in Midstates last season, you will need a registration number to use when logging in that the club will give you at tryouts or shortly after so please wait for that information. You will also need to complete the Mid States Definitive Emergency Medical Care Consent form from this web site.  You will need to print two each of these forms and turn them in to FZW Hockey club be properly registered

For the Mid States developmental league (JVD and C teams) :
